How do I apply for a leave of absence?
You will need to complete a packet of information which you obtain in Human Resources. We contract with a 3rd Party Administrator who reviews and makes the determination if you are eligible for Short-Term Disability. Other leaves are managed through the Human Resources department but may also require your department head approval.
