Delayed Registration of Foreign Birth - Grand Traverse County Probate Court

Residents of Grand Traverse County seeking to establish a State of Michigan Birth Certificate following the adoption of their child in a foreign country can do so by filing for a Delayed Registration of Foreign Birth with the Probate Court.

When filing a Petition for Delayed Registration of Foreign Birth with the Grand Traverse County Probate Court, please use the following checklist for items that must be completed and returned to the Probate Court:

  • Petition and Order for Delayed Registration of Foreign Birth (PCA 350).  Petitioner(s) completes the upper section of this form.  If you were able to change the child’s name in the other country, you must still list the child’s former name AND new name in the area titled “In the Matter of”.  If you were not able to change the child’s name in the other country, and you are requesting a name change, please complete the area titled “In the Matter of” with the child’s current name, and list what the child’s new name will be under number four (4).  Do not write anything in the ‘Order’ area.  
  • Completed Application to Establish Delayed Registration of Foreign Birth.  Complete the front of the form, and sign the back.  The new name of the child should be used.  This will be considered your request for a name change of the child.  Please also complete the bottom right section on the back, indicating how many birth records you would like, along with the amount of the fee you have enclosed.  The fee for the new birth record(s) should be made payable to the State of Michigan in the form of a check or money order.  The Court will send the appropriate documents, and your fee, to the Vital Records Changes office.
  • A copy of the foreign adoption orders
  • A translation of the foreign adoption orders
  • A copy of the foreign birth certificate
  • The translation of the foreign birth certificate
  • An affidavit of the translations (Verification by the translator that the translation is true and correct)
  • Payment of the filing fee: $175, payable to the Grand Traverse County Probate Court. (we do not accept credit cards)  If you would like a certified copy of the Petition and Order for Delayed Registration of Foreign Birth, this is an additional $11.00.
  • Payment to the State of Michigan for the correct fees, depending on how many birth records you are requesting.  (See Application to Establish Delayed Registration of Foreign Birth for fee amounts)  Payment must be made in the form of a check or money order.
Once your Petition and Order for Delayed Registration of Foreign Birth is signed by the Judge, we will then send you a copy of the signed Petition and Order, along with a copy of your completed Application to Establish Delayed Registration of Foreign Birth.  These are copies for your records.  The Court will then send your Petition and Order, Application to Establish Delayed Registration of Foreign Birth, and your fee to the Vital Records Changes office.  The Vital Records office will then create a birth record for your child and mail it to your home address.  The contact information for Vital Records can be found on the top of the Application to Establish Delayed Registration of Foreign Birth.
